Support Coordinator

QUALIFICATIONS: Bachelor Degree from an accredited college or university or a Registered Nurse license. A strong desire and passion for empowering people and social justice. PERSONAL STRENGTHS AND ASSETS: *Understands, believes in and implements the mission, vision and values of Progressive Community Services *Ability to communicate effectively with diverse people, fostering collaboration *Ability to provide support to solve problems and navigate conflict utilizing training and tools *Must maintain high level of confidentiality *Understands and demonstrates the ability to self-direct and meet deadlines *Be a good listener *Engage in ongoing professional development to increase best practice and quality of life for people supported TECHNOLOGY BASED RESPONSIBILITIES: *Use Agency software to track and complete all work within timelines *Utilize existing information on Agency server and websites as well as research for resources that can help people we support reach their goals. Share information with the team. RELATIONSHIP BASED RESPONSIBILITIES: *Use LifeCourse Framework, Personal Outcome Measures and Person-Centered Thinking to support people and families in developing a vision for their life. *Build strong relationships with people supported, their families and people surrounding them. Provide support in discovering peoples' vision, planning, locating and linking to resources to help people reach their goals. Listen to families and people supported to better understand their goals and priorities. *Provide anticipatory guidance as it relates to getting to a persons' "good life" *Contribute to a positive, collaborative and supportive work environment. COMMUNITY BASED RESPONSIBILITIES: *Represent PCS and agency vision with community clubs and memberships and communicate updates to leadership team *Grow knowledge of inclusive community resources that are available to people we support. Share information with Support Coordination team. Notify supervisor(s) of gaps. ELIGIBILITY BASED RESPONSIBILITIES: *Submit timely wavered service requests and funding components as needed for people supported *Monitor funded services to assess quality, timeliness and effects of services and supports. Address issues with support teams and with supervisor as needed *Ensure all documentation, logging records are in accordance to state and federal guidelines as well as PCS policies and procedures *Review event reports and follow abuse neglect guidelines. Follow up on investigations as necessary. *Attend training to remain knowledgeable about Support Coordination requirements and changes. Implement changes as requested.
